WAIVER & RELEASE OF DAMAGES AGAINST THE STATE OF UTAH

PARTICIPATION IN AN APPROVED SPECIAL EVENT ON STATE ROADS

I, do hereby release and discharge the State of Utah, the Department of Transportation Commission, the Utah Highway Patrol, and their officers, agents and employees, along with the City of Oakley and their Committees, from all claim, demands and causes of actions of every and any kind whatsoever for any damages and/or injuries which may result from my participation, or the participation of my child (NAME) in the Oakley 4^th of July Parade (held Thursday, July 4, 2013) involving roads within the Utah State Highway System.
